Résumé
La psychose débutante non traitée occasionne beaucoup de souffrance et son impact se répercute à divers degrés sur le fonctionnement de la personne atteinte. Lorsqu’elle survient dans un milieu de centre-ville, un défi supplémentaire s’impose puisque plusieurs jeunes vivent dans des conditions précaires, instables et sont désaffiliés. Il faut adapter l’organisation des soins pour éviter un retard dans l’identification et le traitement de la psychose. Cet article présente différentes pistes de solution en décrivant la Clinique JAP du CHUM, clinique d’intervention précoce pour la psychose, qui offre des services intégrés visant à optimiser l’évolution. Pour favoriser l’adhésion au traitement, il importe d’offrir un climat accueillant et convivial, d’avoir un cadre d’intervention flexible et de faire preuve d’un espoir optimiste mais réaliste. Des interventions intensives offertes par une équipe interdisciplinaire spécialisée, en collaboration avec les différents acteurs communautaires, doivent être mises en place de façon proactive afin d’abréger le délai du traitement et d’éviter des conséquences désastreuses de la psychose.
Abstract
Untreated psychosis causes a lot of suffering and its impact echoes on different aspects of the affected individual’s functioning. In city centers, youths presenting early psychosis face additional challenges because many of them live in precarious, unstable conditions and are isolated. Reorganisation of mental health care access and delivery is necessary to avoid delay for psychosis identification and treatment initiation. This article describes possible solutions which have been applied at Clinique JAP, a specialized early psychosis intervention clinic at the CHUM, which offers integrated services to optimize outcome. To increase engagement and adherence to treatment, treatment setting must include a welcoming and youth-friendly atmosphere, individualized and flexible interventions, and nourishing optimistic but realistic hope. Intensive interventions by a specialized interdisciplinary team, in collaboration with community organizations, must be offered in a proactive way to reduce delay in treatment and therefore avoid disastrous consequences of psychosis.

La psicosis inicial no tratada ocasiona muchos sufrimientos y su impacto repercute a diversos niveles en el funcionamiento de la persona que la sufre. Cuando surge en un medio del centro de la ciudad, impone un reto suplementario debido a que varios jóvenes viven en condiciones precarias, inestables, de desinserción. Es necesario adaptar la organización de los cuidados para evitar un retraso en la identificación y el tratamiento de la psicosis. Este artículo presenta diferentes pistas de solución al describir la Clínica Para Jóvenes Adultos Psicóticos (JAP) del Centro Hospitalario de la Universidad de Montreal (CHUM), clínica de intervención temprana para la psicosis, que ofrece servicios integrales que buscan optimizar la evolución. A fin de favorecer la adhesión al tratamiento, es importante ofrecer un clima acogedor y convival, contar con un marco de intervención flexible y demostrar esperanza optimista pero realista. Deben ponerse en marcha, de manera más activa, intervenciones intensivas ofrecidas por un equipo interdisciplinario especializado, en colaboración con los diferentes actores comunitarios, a fin de acortar el plazo del tratamiento y evitar las consecuencias desastrosas de la psicosis.

A psicose incipiente não tratada ocasiona muito sofrimento e seu impacto repercute em diversos graus na vida da pessoa atingida. Quando ela ocorre em um meio urbano, um desafio suplementar se impõe, visto que vários jovens vivem em condições precárias, instáveis, sem laços familiares. É preciso adaptar a organização dos cuidados para evitar um atraso na identificação e no tratamento da psicose. Este artigo apresenta diferentes pistas de solução, descrevendo a Clínica JAP do CHUM (Centro Hospitalar da Universidade de Montreal), clínica de intervenção precoce para a psicose, que oferece serviços integrados que visam otimizar a evolução. Para favorecer a adesão ao tratamento, é importante oferecer um clima acolhedor e amigável, ter um quadro de intervenção flexível e demonstrar um espírito otimista, mas realista. As intervenções intensivas oferecidas por uma equipe interdisciplinar especializada, em colaboração com os diferentes profissionais comunitários, devem ser estabelecidas de maneira proativa, a fim de reduzir o prazo do tratamento e evitar as consequências desastrosas da psicose.
