Résumé
Les services à domicile offerts par les CLSC constituent le maillon stratégique des services socio-sanitaires de soutien à domicile. Cet article met en évidence le grave sous-financement de ces services et montre quelles stratégies d'adaptation sont déjà à l'œuvre. L'approche communautaire et un réalignement de l'ensemble des services socio-sanitaires apparaissent à cet égard les seules voies capables de réconcilier les nouvelles contraintes et les valeurs de base. Dans la mesure où, à l'avenir, la nécessité d'améliorer les services offerts va s'imposer, de même que celle de répondre de manière plus diversifiée aux besoins, l'alliance avec le milieu communautaire apparaît incontournable. La promotion des conditions politiques favorisant l'émergence d'une politique intersectorielle de soutien à domicile devrait mobiliser les CLSC.
Abstract
The CLSC's home services are a strategic link with health care and social services which help the community's disabled elderly population. This article will review and take a position in light of the conclusions of certains studies made in Québec and elsewhere. It will show the gap between the department's policies and the means provided to ensure true accessibility of services to the people and will discuss adaptation strategies already being implemented. The community approach and a realignment of all health care and social services seem the only way to reconcile new constraints with the basic aim of these services to improve quality of life for the elderly within the community. In future, it may be necessary to improve the services offered, and to provide a more diversified and consistent response to needs, to forge an alliance with the community, obligatory coordination of various components of the health care and social service system and reallocation of funds to develop these services. Finally, certain policies should be encouraged so that various sectors can produce a single policy on home care which unites the various social participants.
