Résumé
Le trouble de personnalité limite (TPL) est un grave problème de santé mentale dont l’une des principales caractéristiques est de prédisposer à des difficultés relationnelles importantes. Ceci a pour conséquence d’alimenter les attitudes négatives des intervenants qui oeuvrent auprès de cette clientèle difficile et d’augmenter le risque d’impasse dans le traitement. Cet article présente six grandes stratégies qui permettent de limiter l’émergence d’un sentiment d’impasse chez les cliniciens. L’auteur présente les principaux points communs des équipes de traitement qui s’avérent efficaces dans leur travail auprès de cette clientèle. Une histoire de cas illustre que, même dans des conditions optimales, des impasses cliniques se produisent, et que ces impasses peuvent constituer une opportunité de croissance pour le patient et son psychothérapeute. En conclusion, l’auteur recommande que plus de formation sur le TPL soit offerte aux professionnels de la santé afin d’éviter la stigmatisation de cette clientèle, d’améliorer l’organisation des services offerts et d’augmenter l’efficience des traitements disponibles.
Abstract
Borderline personality disorder is a serious mental health problem for which one of its main characteristics is significant difficulties in relationships with others. These relational problems have the unfortunate consequence of fostering negative attitudes among mental health professionals and contributing to the stigmatization of people suffering from this disorder. In this article, the author emphasizes the importance of taking into account the parameter of the therapeutic frame within which the feeling of facing a stalemate in the treatment of borderline personality disorder patients occurs. Six general strategies are presented that enable the therapist to limit or hinder the risk of stalemate in treatment. This article then presents the commonalities between treatments teams that tend to feel comfortable and efficacious in their management of borderline personality disorder patients. Finally, a case history is used to illustrate how some stalemates can in fact be seen as opportunities for growth for both the patient and the therapist. In order to avoid the vicious circle of negative interactions with patients already hypersensitive to inconsistencies and rejection, the author concludes by insisting on the necessity that more mental health professional have access to training programs and workshops specifically addressing how to better manage and treat people with BPD.

El trastorno de la personalidad límite (TPL) es un grave problema de salud mental en el que una de las principales características es predisponer a las dificultades relacionales importantes. Esto tiene por consecuencia la alimentación de actitudes negativas por parte de los interventores que trabajan con esta clientela difícil y el acrecentamiento del riesgo de llegar a callejones sin salida en el tratamiento. Este artículo presenta seis grandes estrategias que permiten limitar el surgimiento de un sentimiento, por parte de los clínicos, de haber llegado a un callejón sin salida. El autor presenta los principales puntos comunes de los equipos de tratamiento considerados como eficaces en su labor con esta clientela. Una historia de caso ilustra que, incluso en condiciones óptimas, se producen callejones sin salida y que estos callejones pueden constituir una oportunidad de crecimiento para el paciente y el psicoterapeuta. En conclusión, el autor recomienda una mayor formación en TPL para los profesionales de la salud a fin de evitar la estigmatización de esta clientela y acceder a una mejor gestión y mejores tratamientos.
Resumo
O transtorno de personalidade limite (TPL) é um grave problema de saúde mental, no qual uma das principais características é a predisposição a dificuldades relacionais importantes. Essa predisposição tem por consequência alimentar atitudes negativas dos intervenientes que trabalham junto a esta clientela difícil e aumentar o risco de impasse no tratamento. Este artigo apresenta seis grandes estratégias que permitem limitar o aparecimento de um sentimento de impasse nos médicos clínicos. O autor apresenta os principais pontos comuns das equipes de tratamento que se estimam eficazes em seu trabalho junto a esta clientela. Uma história de caso ilustra que, mesmo em condições ideais, impasses clínicos se produzem, e que esses impasses podem constituir uma oportunidade de crescimento para o paciente e para o psicoterapeuta. Em conclusão, o autor recomenda mais formação sobre o TPL para os profissionais da saúde, a fim de evitar a estigmatização desta clientela e chegar a uma melhor gestão e melhores tratamentos.
