Résumé
Cet article décrit le processus d’implantation du programme interne de réhabilitation intensive de l’hôpital Douglas destiné aux personnes souffrant de troubles mentaux graves. La discussion des paradigmes contemporains de la réhabilitation sous-tend l’intégration des perspectives biologique et psychosociale dans la caractérisation et le traitement de la pathologie. La discussion de la réorganisation de l’ensemble des services de réhabilitation du Programme des troubles mentaux graves conduit à spécifier le contexte organisationnel et régional de l’implantation. Les axes prioritaires d’implantation sont l’analyse des besoins, la formation, la réorganisation du travail et la modification de l’environnement de soins. Ces activités sont mises en oeuvre en référence aux développements scientifiques empiriques pertinents et font l’objet d’une évaluation continue. Les défis de l’avenir portent sur la poursuite de l’intégration des modèles de soins, des milieux et des services dans le contexte de la nouvelle hiérarchisation des soins de santé mentale au Québec.
Abstract
This article describes the process of implementation of an internal intensive rehabilitation program at Douglas hospital for people with severe mental disorders. A discussion of contemporary paradigms of rehabilitation implies the integration of biological and psychosocial perspectives in the characterization and treatment of the pathology. The discussion of the reorganization of the entire rehabilitation services of the Program for severe mental disorders leads to specifying the organizational and regional context of the implementation. The axes of implementation in priority are the following : analysis of needs, training, reorganization of work and modification of the setting of care. These activities are implemented in reference to scientific developments that are empirically relevant and assessed regularly. Future challenges include the pursuit of integration of treatment models, of settings and services within the context of the hierarchy of mental health care in Québec.

Este artículo describe el proceso de implantación del programa interno de rehabilitación intensiva del hospital Douglas, destinado a las personas que sufren de trastornos mentales graves. La discusión de los paradigmas contemporáneos de la rehabilitación sirve de base a la integración de las perspectivas biológicas y psicológicas en la caracterización y tratamiento de la patología. La discusión de la reorganización del conjunto de servicios de rehabilitación del Programa de trastornos mentales graves conduce a especificar el contexto organizacional y regional de la implantación. Los ejes prioritarios de la implantación son el análisis de las necesidades, la formación, la reorganización del trabajo y la modificación del medio de atención. Estas actividades se ponen en práctica en relación con los desarrollos científicos empíricos pertinentes y son objeto de una evaluación continua. Los retos del provenir tienen por objeto la búsqueda de la integración de los modelos de atención, los medios y los servicios en el contexto de la nueva jerarquización de la atención de la salud mental en Quebec.

Este artigo descreve o processo de implantação do programa interno de reabilitação intensiva do hospital Douglas destinado às pessoas que sofrem de transtornos mentais graves. A discussão dos paradigmas contemporâneos da reabilitação subentende a integração das perspectivas biológica e psicossocial na caracterização e no tratamento da patologia. A discussão da reorganização de todos os serviços de reabilitação do Programa dos Transtornos Mentais Graves leva a especificar o contexto organizacional e regional da implantação. Os eixos prioritários de implantação são a análise das necessidades, a formação, a reorganização do trabalho e a modificação do ambiente de atendimento. Estas atividades são criadas em referência aos desenvolvimentos científicos empíricos pertinentes e são objeto de uma avaliação contínua. Os desafios do futuro tratam sobre a continuação da integração dos modelos de atendimento, dos meios e dos serviços no contexto da nova hierarquização do atendimento de saúde mental no Quebec.
