Résumé
La prévalence du trouble de personnalité limite en clinique externe de psychiatrie, la sévérité des symptômes présentés dont la mortalité potentielle et la difficulté de traiter ces patients souvent récalcitrants justifient amplement un guide de lignes directrices de pratique sur leur traitement. Grâce à une expérience non négligeable acquise dans la création de treize guides de lignes directrices de pratique, l’American Psychiatric Association a produit un guide qui répond assez bien aux exigences scientifiques d’un guide de pratique et qui satisfait presque tous les critères souhaités par l’Association médicale canadienne et le comité aviseur des Services de santé publique de l’Institut de médecine américain. L’article décrit les conseils sur le traitement psychothérapeutique et pharmacologique, et les données probantes qui soutiennent ces recommandations. L’article commente et critique aussi le guide.
Abstract
The prevalence of patients with borderline personality disorder in outpatient psychiatric clinics, the severity of their presenting symptoms including potential mortality and the difficulty to treat these poorly collaborative healthseekers are sufficient arguments to justify the need for a practice guideline about their treatment. Supported by a strong experience in the creation of many practice guidelines, the American Psychiatric Association has produced a guide that answers quite well to the scientific criteria of a treatment guide and respects most of the specified requirements emitted by the Canadian Medical Association and the Advisory Committee to Public Health Services of the American Institute of Medicine. A review of the suggested recommendations on psychotherapeutic and pharmacological treatments and their scientific evidence is given followed by comments and a critical discussion.

La prevalencia del trastorno de personalidad límite en la clínica de psiquiatría externa, la severidad de los síntomas presentados, como la mortalidad potencial y la dificultad de tratar estos pacientes con frecuencia recalcitrantes, justifican ampliamente una guía de directrices de práctica sobre su tratamiento. Gracias a una experiencia considerable, adquirida durante la creación de trece guías de directrices de práctica, la Asociación Americana de Psiquiatría (American Psychiatric Association) produjo una guía que responde suficientemente bien a las exigencias científicas de una guía de práctica y que cumple con casi todos los criterios de la Asociación Canadiense de Medicina (Association médicale canadienne) y el comité de consejo en Servicios de Salud Pública del Instituto Americano de Medicina (comité aviseur des Services de santé publique de l’Institut de médecine américain). El artículo describe los consejos sobre el tratamiento psicoterapéutico y farmacológico y los datos concretos que apoyan estas recomendaciones. El artículo comenta y critica también la guía.
Resumo
A prevalência do problema de personalidade borderline em clínica externa de psiquiatria, a gravidade dos sintomas apresentados, entre outras a mortalidade potencial, e a dificuldade de tratar estes pacientes sempre rebeldes justificam largamente a criação de um manual de linhas diretrizes de prática sobre o tratamento. Graças a uma experiência considerável, adquirida pela criação de treze manuais de linhas diretrizes de prática, a American Psychiatric Association produziu um manual que responde bem às exigências científicas de um manual de prática e que satisfaz quase todos os critérios procurados pela Associação Médica Canadense e pelo comitê consultivo dos Serviços de Saúde Pública do Instituto de Medicina Americano. O artigo descreve os conselhos sobre o tratamento psicoterapêutico e farmacológico desta doença, e os dados probantes que apóiam estas recomendações. O artigo também comenta e critica o guia.
