RÉSUMÉ
Cette étude se situe dans le contexte de la réorganisation des services de santé et des services sociaux au Québec et vise à décrire les caractéristiques physiques et mentales des personnes demeurant au centre hospitalier psychiatrique. Dans la première étude, 146 personnes présentant une déficience intellectuelle dont 74 forment le groupe en externe (dans la communauté) et 72, le groupe interne (hospitalisé). Les résultats indiquent que 84 % des personnes présentant une déficience intellectuelle demeurant en institution présentent des besoins suffisamment importants aux trois variables santé, déficits et comportements pour justifier un encadrement plus intensif. La variable qui influence le plus l'intégration sociale est celle des comportements préjudiciables. Comme les personnes demeurant en institution et qui devront éventuellement intégrées la communauté, ont un degré d'incapacité élevé et des comportements préjudiciables plus nombreux, l'encadrement devra être soutenu et intensif auprès des sujets et des responsables de services. Dans la deuxième étude, 928 personnes en milieu institutionnel souffrant de troubles mentaux ont été étudiés selon les variables, âge, sexe, diagnostic et les scores agglomérés de l'Inventaire du niveau de soins et l'autonomie fonctionnelle. Les résultats démontrent que la proportion de femmes de 35 ans et plus croit progressivement par rapport aux hommes. La répartition des diagnostics est principalement la schizophrénie se situant approximativement à 70 %. L'âge devient un facteur important dans l'évaluation des soins physiques des personnes souffrant de problèmes mentaux. La population interne est vieillissante et les femmes sont de plus en plus nombreuses dans ces groupes d'âge. Ces personnes présentent davantage des troubles physiques et une autonomie déclinante. En vertu du soutien requis par ces personnes au moment de l'intégration dans la communauté, on devra offrir des ressources plus structurées et plus spécialisées pour répondre à leurs besoins.
ABSTRACT
Within the context of the general reorganization of health care and social services in Québec, the present study aims at describing physical and mental characteristcis of persons with mental health problems and who are mentally retarded still hospitalized in a psychiatric hospital. In the first study, 146 mentally retarded persons of which 74 formed the community group and 72 the institutionalized group. The results indicate that 84 % of the mentally retarded persons still hospitalized manifested sufficiently important needs on the three measured variables (health, deficits and behaviors) to justify intensive care in a structured environment. The Behavior variable is more important in deciding the integration of these people in the community. As most of these persons have a higher level of disabilities and more behavior manifestations, the support required for these people in the community and persons delivering services will have to be more structured and intensive in nature. In the second study, 928 psychiatric patients still hospitalized were studied on the variables, age, sex, diagnostics and the global scores (physical and mental) of the level of care survey and functional autonomy. The results show that the proportion of women in the age group 35 and older were progressively increasing in relation to the men. The primary diagnostic reported more frequently was schizophrenia in 70 % of cases. In the evaluation of physical care of the people still hospitalized, age becomes an important factor. Generally speaking, the population within the institution is aging and women are progressively growing in number. As these people expressed more physical ailments and a decline in basic autonomy, more structured and specialized care and support will be required to respond to their needs.
