Résumé
Avec l’arrivée de la pandémie en mars 2020, le Québec a restreint de façon significative la présence des proches auprès des patients dans le réseau de la santé et des services sociaux (RSSS). Plusieurs personnes en fin de vie sont décédées seules, sans confort, soins de base, ni accompagnement, dans des conditions discordantes avec la philosophie des soins palliatifs.
L’intervalle de temps entre les deux premières vagues de la pandémie a permis au gouvernement de réaliser un exercice rétrospectif sur la question. Le ministère de la Santé et des Services sociaux (MSSS) invitait alors à une plus grande contextualisation des directives émises sur les visites, reconnaissant l’importance et le rôle que jouent les proches dans la vie et les soins aux patients.
Appuyées par les mots prononcés par des patients, des proches, des membres du personnel soignant et des gestionnaires, nous partageons nos réflexions et observations en tant qu’éthiciennes cliniques. Plus précisément, nous présentons trois enjeux éthiques liés à la présence des proches : l’iniquité découlant de l’interprétation variable des directives, la détresse morale et l’entrave aux valeurs des soins de fin de vie. Nous proposons une approche intégrée des valeurs et abordons la communication comme source de mitigation des enjeux. Nous présentons enfin en quoi les services en éthique clinique peuvent soutenir les équipes, les gestionnaires, les proches et les patients dans la résolution des enjeux liés aux visites.

Abstract
With the arrival of the pandemic in March 2020, Quebec significantly restricted the presence of family members with patients in the health and social services network. Many people at the final phase of life died alone, without comfort, basic care, or accompaniment, in conditions that are inconsistent with palliative care’s philosophy.
The gap between the first two waves of the pandemic allowed the government to conduct a retrospective examination of the issue. The ministère de la Santé et des Services sociaux (MSSS) called for a greater contextualization of the guidelines issued on the visits, recognizing the importance and the role that relatives play in the life and care of patients.
Based on the comments of patients, loved ones, caregivers, and management, we share our thoughts and observations as clinical ethicists. Specifically, we analyze three ethical issues related to the presence or absence of relatives: inequity arising from varying interpretations of directives, moral distress, and the blocking of values for end-of-life care. We propose an integrated approach and discuss how communication can mitigate these issues. Finally, we present how clinical ethics services can support end-of-life health professionals, management, relatives, and patients in resolving issues surrounding visits.

Resumen
Con la llegada de la pandemia en marzo de 2020, Quebec restringió considerablemente la presencia de familiares con pacientes en la red de servicios sanitarios y sociales. Muchas personas al final de la vida han muerto solas, sin consuelo, sin cuidados básicos ni acompañamiento, en condiciones que no se corresponden con la filosofía de los cuidados paliativos.
El intervalo de tiempo entre las dos primeras oleadas de la pandemia permitió al gobierno realizar un ejercicio de retrospección sobre el tema. El ministère de la Santé et des Services sociaux (MSSS) pidió una mayor contextualización de las directivas emitidas sobre las visitas, reconociendo la importancia y el papel que los familiares desempeñan en la vida y el cuidado de los pacientes.
Apoyadas en las palabras de pacientes, familiares, cuidadores y gestores, compartimos nuestras reflexiones y observaciones como éticistas clínicas. En concreto, presentamos tres cuestiones éticas relacionadas con la presencia de los familiares: la inequidad derivada de la interpretación variable de las directivas, la angustia moral y la interferencia con los valores de los cuidados al final de la vida. Proponemos un enfoque integrado de valores y debatimos la comunicación como fuente de mitigación de los problemas. Por último, presentamos cómo los servicios de ética clínica pueden apoyar a los equipos, a los gestores, a los familiares y a los pacientes en la resolución de los problemas relacionados con las visitas.
